Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
We accept most insurance plans, however if we are not accepting your insurance at this time we can provide you with the necessary treatment information so that you may file a claim with your personal health insurance company if you so desire.
Cancellations must be made 24 hours in advance. If you cancel an appointment without giving 24 hour notice you will be charged $25.00 and must pay this fee before scheduling a new appointment. Please note that missed or canceled appointments are not covered by insurance.
